This opportunity offers a 13-week contract, serving patients in their homes with a Monday through Friday, 8:00 AM to 5:00 PM schedule. The position involves managing a caseload of approximately 25–32 points per week, typically translating to 5–7 patient visits daily, depending on the type of care provided. Leveraging tools like Home Care Home Base (HCHB) for electronic documentation and OASIS for patient assessments, you will help clients regain mobility, safety, and independence in a field-based role with daily travel within your assigned territory (mileage reimbursement provided).
