Schedule/Hours:
• Travel assignment – flexible schedule tailored to facility needs

Responsibilities:
• Perform evaluative and therapeutic procedures under physician guidance
• Create, implement, and modify therapy plans of care based on resident goals
• Document all patient interactions in the Electronic Medical Record
• Consult with care teams regarding treatment plans and discharge needs
• Educate residents and caregivers on therapeutic techniques and assistive devices
• Mentor new team members and support assistant-level staff
• Stay current with clinical best practices and compliance standards
• Promote overall cleanliness and comply with infection control procedures

Desired Qualifications:
• Bachelor’s degree required; Master’s or Doctorate preferred
• Current state licensure as a Physical Therapist
• Previous experience in long-term care or skilled nursing preferred
• Basic Life Support (BLS) certification preferred
• Excellent communication, time management, and teamwork skills
• Proficiency in Microsoft Office and electronic documentation
