Deliver high-quality, patient-centered physical therapy services in a home health setting, supporting clients in regaining their independence, mobility, and strength. This full-time, 13-week contract offers a dynamic field-based opportunity, working with patients in their home environments and collaborating within an interdisciplinary healthcare team.

Schedule:

  • Monday – Friday
  • 8:00 AM – 5:00 PM
  • Responsibilities:

  • Perform comprehensive physical therapy evaluations, utilizing OASIS assessments for Start of Care, Resumption, Recertification, and Discharge as needed.
  • Design and execute tailored therapy plans focusing on functional mobility, pain management, fall prevention, and patient safety.
  • Deliver skilled interventions including therapeutic exercise, gait and transfer training, neuromuscular re-education, and balance activities.
  • Educate patients and caregivers on exercise routines, the safe use of assistive devices, and prevention strategies while promoting active participation in the rehabilitation process.
  • Document all patient care promptly and thoroughly using the Home Care Home Base (HCHB) electronic system.
  • Collaborate with nurses, OT, SLP, social workers, and aides to deliver cohesive, coordinated care and participate in care conferences and interdisciplinary meetings.
  • Maintain adherence to agency policies, professional standards, and Medicare home health guidelines.
  • Travel to patient homes within the assigned territory, with mileage reimbursement provided.
  • Qualifications:

  • Active Physical Therapist (PT) license (in good standing) within the state of practice.
  • At least one year of recent home health physical therapy field experience required.
  • Experience with OASIS assessments and proficiency in Home Care Home Base (HCHB) strongly preferred.
  • Reliable transportation, valid driver’s license, and current auto insurance.
  • Excellent clinical judgment, communication, and time management skills.
  • Ability to work independently with a flexible, field-based schedule.
