Deliver high-quality care in a home health setting by providing physical therapy services to adult patients with chronic disease management and wound care needs. This contract opportunity presents the chance to work day shifts, supporting individuals in their recovery while collaborating with a skilled interdisciplinary team.

Schedule:

  • 8-hour day shifts, Monday through Friday (8:00 AM – 5:00 PM)
  • No on-call, call back, or weekend rotation required
  • Holiday coverage as scheduled
  • Qualifications and Experience:

  • Active Physical Therapist (PT) license in the state
  • BLS certification
  • Minimum of one year of experience, with strong prior work in home health preferred
  • Comfortable with adult and geriatric patient populations, especially those managing chronic disease, wounds, IVs, pleurx, and wound vacs
  • Charting experience using Homecare Homebase (required) and fluency with OASIS-E assessment
  • Strong assessment, wound care, teaching, and maintenance notes documentation skills
  • Valid driver’s license and proof of insurance
  • Role Responsibilities:

  • Conduct home visits to adult patients (approx. 6 points per day)
  • Complete comprehensive assessments and documentation using Homecare Homebase EMR
  • Deliver hands-on physical therapy, collaborate with leadership, and provide education to patients and caregivers
  • Participate in scheduled holiday coverage
  • Interface professionally with patients, families, and other clinicians
