A rewarding opportunity is available for an experienced, compassionate clinician to deliver quality physical therapy care in the home health setting. This contract assignment requires a motivated professional ready to work independently while providing patient-centered services that help individuals regain strength, balance, and independence.

Schedule:

  • Full-time, 13-week contract
  • Monday – Friday, 8:00 AM – 5:00 PM
  • Key Responsibilities:

  • Perform comprehensive physical therapy evaluations, utilizing OASIS assessments when required.
  • Design and implement individualized plans of care focused on functional progress in mobility, strength, gait, pain management, and safety.
  • Deliver skilled interventions, including therapeutic exercise, neuromuscular re-education, transfer and gait training, and fall prevention.
  • Monitor outcomes, adapt treatment strategies based on patient status, and communicate effectively with healthcare team members.
  • Educate patients and caregivers on therapeutic exercises, safety awareness, assistive devices, and home exercise programs.
  • Maintain timely, accurate documentation in the Home Care Home Base (HCHB) electronic system.
  • Collaborate with the interdisciplinary care team to support positive outcomes and coordinated care.
  • Participate in team meetings, case conferences, and required professional development activities.
  • Adhere to agency policy, Medicare guidelines, and regulatory standards.
  • Desired Qualifications:

  • Active Physical Therapist license (in good standing) for state of practice
  • At least one year of recent Home Health PT field experience
  • Proficiency with Home Care Home Base (HCHB) preferred
  • Required experience with OASIS documentation and assessment
  • Ability to manage a self-directed, field-based schedule
  • Strong clinical reasoning, organization, and communication skills
  • Reliable transportation, valid driver’s license, and insurance
