A rewarding opportunity is available for a dedicated Home Health Physical Therapist Assistant (PTA) to provide compassionate, patient-centered therapy services in a supportive and field-based environment. This full-time, 13-week contract position focuses on delivering skilled rehabilitative care to patients in their homes, prioritizing individualized treatment and improving functional independence.

Schedule:

  • Monday through Friday, 8:00 AM – 5:00 PM
  • Caseload of approximately 25–32 points per week (averaging 5–7 patient visits daily depending on visit types)
  • Daily travel within an assigned local territory, with mileage reimbursement provided
  • Key Responsibilities:

  • Implement tailored physical therapy interventions based on the supervising Physical Therapist’s plan of care
  • Guide patients through therapeutic exercises, gait and balance training, transfer activities, and other tasks to promote mobility and safety
  • Monitor and document patient progress and responses to therapy using Home Care Home Base (HCHB)
  • Communicate status updates, clinical findings, and condition changes to the supervising PT and interdisciplinary care team
  • Educate patients and caregivers on home exercise programs, equipment use, fall prevention, and safety strategies
  • Collaborate with nurses, occupational therapists, social workers, and others for coordinated, high-quality care
  • Maintain accurate, timely documentation and comply with all regulatory and agency guidelines
  • Participate in case conferences and team meetings as needed to support patient outcomes
  • Qualifications:

  • Active, unencumbered PTA license in state of practice
  • At least one year of recent home health PTA experience (field experience preferred)
  • Proficiency with Home Care Home Base (HCHB) and understanding of OASIS processes preferred
  • Strong clinical judgment, interpersonal, and time management skills
  • Reliable transportation, valid driver’s license, and insurance required
