A skilled and compassionate Home Health Physical Therapist is needed to deliver patient-centered care in a field-based role. This contract position will allow you to make a direct impact on patient lives by supporting their functional independence within their homes.

Schedule:

Role Overview: You will complete comprehensive patient evaluations—including OASIS assessments—develop and implement individualized care plans, and provide skilled physical therapy interventions such as gait training, transfer training, and therapeutic exercise. Daily responsibilities involve collaborating closely with interdisciplinary healthcare team members, educating patients and caregivers on safety and exercise, and maintaining accurate documentation using Home Care Home Base (HCHB). Expect to see approximately 25–32 points weekly, with 5–7 visits per day, and to regularly travel within your assigned territory (mileage reimbursement provided).

$2,253 - $2,383/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
