Embark on a fulfilling opportunity providing home health physical therapy services to adult patients with chronic conditions and orthopedic diagnoses. In this contract role, you’ll leverage your clinical expertise while working independently yet supported by a collaborative team. You'll travel throughout Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and nearby communities.

Schedule/Hours:

  • No weekend rotations
  • No on-call requirements
  • No major holiday shifts
  • Rotating schedule cycle every 13 weeks
  • Key Qualifications:

  • At least 1 year of experience delivering home health therapy
  • State Physical Therapy licensure in good standing
  • BLS certification
  • Active driver’s license and proof of insurance
  • Comfortable traveling an average of 60-80 miles daily (mileage reimbursed weekly)
  • Seasoned in home health environments (first-time travelers welcome with relevant experience)
  • Strong organizational skills to independently schedule daily visits
  • Primary Responsibilities:

  • Conduct six therapy visits per day, providing patient-centered care in the home setting
  • Assess, develop, and implement individualized treatment plans for adults with chronic illnesses and post-operative orthopedic needs
  • Utilize EPIC Rover (view only), Focura, Samsung Note 9 devices, and Home Care Home Base to document care and communicate effectively
  • Collaborate with a robust support staff including schedulers, department leadership, clinical managers, supervisors, and peer clinicians
  • Coordinate appointment schedules directly with patients and families, ensuring clear communication prior to each visit
  • Adhere to departmental standards, including maintaining at least 30 visit equivalents per week
