Step into an impactful physical therapy role serving a rural community and supporting geriatric patients in a hospital home health setting. This contract position offers a structured weekday schedule and the chance to collaborate with a dedicated therapy team. You’ll complete visits, evaluations, admissions, and discharges while helping patients maximize their mobility and quality of life.

Schedule:

  • Monday through Friday, 8:00 am – 5:00 pm
  • No weekend or on-call requirements
  • Holiday shifts scheduled as needed
  • Qualifications:

  • Active and unrestricted PT license
  • Current CPR certification
  • Valid driver’s license and car insurance for field visits
  • Experience with geriatric populations preferred
  • Comfort using EMR (specifically HCHB platform)
  • Ability to cover branch areas as needed (within specified counties)
  • Primary Responsibilities:

  • Provide home health therapy to an average of 6 patients daily
  • Conduct thorough patient evaluations, perform visits, manage admissions and carry out discharges
  • Document care and progress accurately in the EMR
  • Work collaboratively with a team of PTs and PTAs to serve your assigned caseload
  • Communicate effectively with patients, families, and other care providers
  • Adapt to branch coverage as patient needs dictate
  • Attend comprehensive 5-day orientation prior to independent practice
  • Follow required dress code (light gray scrubs)
