  • Key Responsibilities:

  • Conduct comprehensive patient assessments and develop, implement, and adjust individualized treatment plans.
  • Direct discipline-specific therapy services, delegating and collaborating with team members as appropriate.
  • Ensure care delivery aligns with physician directives, regulatory standards, and best practices focused on patient safety.
  • Maintain clear, accurate, and timely medical documentation, including functional quality indicators and letters of medical necessity.
  • Educate patients, families, students, and colleagues, fostering a positive learning environment and upholding high standards of care.
  • Proactively pursue ongoing professional growth and participate in student programs and organizational performance improvement initiatives.
  • Adhere to state/federal guidelines, organizational mission, and ethical codes of conduct.
  • Qualifications:

  • Current licensure as a Physical Therapist (PT) in applicable state (or eligibility).
  • Recent hospital-based therapy experience, ideally in an outpatient setting.
  • Strong competency in clinical reasoning, patient assessment, and interdisciplinary communication.
