Schedule/Hours:

  • Travel assignment (specific hours may vary based on assignment needs)

    • Qualifications:

  • Graduation from a physical therapy program accredited by the American Physical Therapy Association or related accrediting agency
  • A current, unencumbered physical therapy license for the state of assignment
  • Preferred: 2-3 years of direct physical therapy experience
  • Current CPR certification
  • Proficiency in using computers for documentation, online training, and HR systems
  • Reliable transportation, valid driver’s license, and required liability insurance

    • Primary Responsibilities:

  • Evaluate patients and develop individualized treatment plans in alignment with physician orders
  • Deliver therapeutic interventions, monitor progress, and document patient outcomes
  • Supervise and evaluate assigned Physical Therapy Assistants and Home Health Aides as appropriate
  • Communicate proactively with physicians and multidisciplinary teams regarding patient status and treatment modifications
  • Educate and instruct patients, families, and care team members for optimal home therapy care and safety
  • Participate in periodic office meetings, in-service education, and professional development opportunities
  • Ensure timely coordination of care transitions and discharge planning
