A leading healthcare facility in California is seeking an experienced Physical Therapist to provide exceptional care in a hospital-based home health setting. This contract role offers an opportunity to work with a diverse patient population, collaborating with a skilled interdisciplinary team.

Schedule/Hours:

Contract position; specific schedule details provided during the interview process

Qualifications:

Active California Physical Therapist license required

Current Basic Life Support (BLS) certification

At least 2 years of recent experience in physical therapy within a hospital or home health environment

Strong clinical evaluation and treatment planning skills

Excellent communication and documentation abilities

Ability to work independently and within a multidisciplinary team

Key Responsibilities:

Assess patients' functional abilities and develop individualized treatment plans

Deliver hands-on therapy services in accordance with physician orders and home health standards

Set and review meaningful goals to ensure patient progress

Educate patients and families on exercises and therapeutic techniques for continued progress at home

Maintain accurate, timely patient documentation in compliance with hospital and regulatory requirements

Coordinate with nursing and other therapy disciplines to achieve best patient outcomes
