In this hospital-based role, you’ll advance your physical therapy practice by collaborating with multidisciplinary teams and gaining insight into multiple state-specific therapy standards. Key Responsibilities:

  • Perform patient evaluations and provide therapeutic services per physician orders.
  • Develop and update individualized treatment plans in compliance with clinical protocols.
  • Observe, record, and report on patient progress to physicians and update health records.
  • Coach and supervise Physical Therapy Assistants and Home Health Aides, ensuring high-quality patient care.
  • Offer guidance and in-service education to family members and agency personnel.
  • Evaluate home environments and recommend modifications for therapy effectiveness.
  • Ensure patient care aligns with state and agency regulations.
  • Travel to patient homes, attend periodic office meetings, coordinate care with an interdisciplinary team.
  • Qualifications:

  • Graduation from an accredited physical therapy program.
  • Active, unencumbered physical therapy license for assignment state(s).
  • Minimum 2-3 years of experience in physical therapy, hospital or home health preferred.
  • Current CPR certification.
  • Proficiency with computers and ability to manage online platforms.
  • Reliable transportation, valid driver’s license, agency-required automobile liability insurance.
